If you need to cancel an appointment, please do so as far in advance as possible. Please bring all insurance information including insurance cards and, if necessary, your HMO referral. Every effort is made to see you at your appointment date and time but occasional emergencies or deliveries may cause delays. We will do our best to keep you from being inconvenienced.

Billing and Insurance:
Payment in full is expected at the time of your visit. We are a participating member of most area insurance plans and are happy to assist you with insurance claims. As a financial courtesy, we will wait for payment from your insurance company, however, any copayment, deductible or fee for uncovered service, and any previous balance not paid by your insurance company is your responsibility at the time of service. If you are participating with any PPO or HMO insurance plan, you will be responsible for the co-pay associated with your plan. Please select the "Billing/Insurance" link for detail.

Medical Records:
Medical records reproduction requests require a fee of $25 for the first 20 pages and $.15 for each additional page. Reproductions are processed within 7-10 days from the date of request. For "rush" requests (next business day instead of regular mail), an additional $15 will be charged. Pre-payment for this service is required.

Prescription Refills:
Notice of 3 business days is required for prescription refill approvals. In case of emergency our office may approve in less than days. To expedite your prescription refill, please use the "prescription refill" link.

Appointments & Scheduling:
To serve you in the best way possible, we work by appointments. We suggest that you call for a routine appointment four to six weeks prior to the time you wish to be seen. Please always let us know of symptoms or issues that may indicate that you need to be seen sooner.

We do our best to keep to appointed schedules, but medical emergencies and unexpected surprises do occur. If that happens and it delays your appointment, we promise to keep you informed and let you know of other options to fit your needs. Always feel free to call ahead to check for any known delays before coming in for your appointment.
